The following procedure is to be followed in completing these forms: Accident forms are to be obtained from the General office. A signed statement must be obtained from the INJURED. A signed statement must be obtained from two or more WITNESSES. A signed statement must be obtained from the TEACHER IN CHARGE of the activity. All reports must be filed within 24 hours of the accident. HALL AND STAIRCASE SAFETY Every teacher should assume an active post in the CENTER of the hall outside his or her classroom during the change of periods. Plan your instruction so that you can be out in the hallway at the end of one and the beginning of the next period. If your classroom is near a staircase, please make a habit of monitoring the students and traffic on the stairs. The administrative staff will also monitor the hallways during the change of periods. We must all encourage students to "WALK TO THE RIGHT." Together we can establish and maintain a positive and safe environment. |
